The Wegovy price without insurance is $349 a month for the pen if you buy through Novo Nordisk's self-pay program (checked 10/18/2026). That pays for one box of 4 pens, a 28-day supply, at any strength from 0.25 mg to 2.4 mg.
Two other prices are worth knowing. New self-pay patients pay $199 for each of their first two fills at the two lowest strengths. The tablet, sold as the Wegovy pill, starts at $149 a month.
Then there's the list price: $1,349.02 a package. It's the number in the headlines, but it isn't what a cash buyer pays through Novo's program.
How much does each Wegovy pen and pill cost?
Novo sets one cash price for each form and strength. NovoCare Pharmacy ships it free, or you can use the self-pay offer to pick it up at a local pharmacy.
The standard pen price used to be higher. Novo cut it on 11/17/2025, effective that day: $499 $349 changed 11/17/2025
Buying more at once doesn't save you anything. Novo's self-pay terms price two months at $698 and three months at $1,047. That's exactly two and three times $349.
The $199 intro price comes with limits. It covers only the 0.25 mg and 0.5 mg pens, only your first two fills, and the prescription has to be filled by 12/31/2026.
Why is the Wegovy list price $1,349.02?
The list price is what the manufacturer charges wholesalers before rebates and discounts. NovoCare shows the same $1,349.02 per package for the pen, the HD pen and the pill (checked 10/18/2026).
If you pay cash through Novo, you can mostly ignore it. If you have insurance with a deductible or coinsurance, it can matter more. Novo itself says its upcoming cut is aimed at people whose out-of-pocket costs are linked to list price
.
That cut is already scheduled. On 02/24/2026, Novo announced a list price of $675 a month for the 2.4 mg pen and 25 mg tablets, starting 01/01/2027: $1,349.02 $675 from 01/01/2027
The same release says the change won't affect self-pay prices. So $349 stays $349, at least by Novo's own account.
How much is Wegovy per milligram?
Per-mg math shows how much medicine each dollar buys. It isn't a reason to pick a strength. That's your prescriber's call.
The pen price is flat, so the low strengths cost far more per mg. The FDA label lists 4 single-dose pens per carton, so a box holds four times the pen's strength.
At 0.25 mg, $349 buys 1 mg of semaglutide. At 2.4 mg, the same $349 buys 9.6 mg. The HD pen costs $50 more and holds three times as much.
We left the pill out of this table. Its doses are much larger in mg than the weekly shots, so putting the two side by side would mislead you.
For the same math on Zepbound and Mounjaro, see tirzepatide cost per mg.
Can you use the Wegovy savings card without insurance?
No. The savings card is for people whose commercial plan covers Wegovy, and it takes off no more than $100 per one-month fill (down to $25 at best). The full rules are in our Wegovy savings card guide.
Do the math before you use it. Say your copay is $150: the card takes off $100 and you pay $50. Now say your plan bills you $600 because you haven't met your deductible. The card still takes off only $100, and the $349 cash price beats your $500 share. One trade-off: under Novo's terms the $349 cash price won't count toward your deductible, while the $600 your plan bills you would.
Coverage can also hinge on prior authorization. That means your prescriber has to show the plan you meet its rules before it pays. Your plan's drug list may be run by a pharmacy benefit manager, a company the plan hires to handle prescription benefits. If so, that's who reviews the request.
If your plan doesn't cover Wegovy at all, the self-pay price is the offer meant for you. Novo's terms open it to people with insurance that does not cover Wegovy®, or those who are self-paying/opting to process outside of their insurance plan
.
That cash purchase stays outside your insurance. Novo's self-pay terms say you agree not to seek reimbursement from any insurer, and that you will not apply those costs toward any deductible or true out-of-pocket requirements
.
The same terms list a health care reimbursement account
among the payers you can't claim the cost back from. NovoCare Pharmacy says we accept payment from FSA/HSA funds
, so if you want to use that money, pay with the HSA or FSA card at checkout. Don't plan on filing a reimbursement claim afterwards.
What does Wegovy cost on Medicare?
$50 a month for people who qualify under the Medicare GLP-1 Bridge, which runs from 07/01/2026 to 12/31/2027 (checked 10/18/2026). Your prescriber has to get prior authorization through the Bridge, and Novo's savings card and self-pay offer don't apply to a prescription billed to Medicare.
Eligibility rules and how to apply are in does Medicare cover Wegovy.
Is Wegovy cheaper on TrumpRx?
No, it's the same price. TrumpRx lists the Wegovy pen from $199 a month and the pill from $149, each against an "original" price of $1,349 (checked 10/18/2026).
Its Wegovy pen page spells out the same deal NovoCare offers. New patients pay $199 for their first two fills of 0.25 mg or 0.5 mg, through 12/31/2026. After that it's $349 a month, or $399 for the 7.2 mg HD pen.
The difference is how you buy. TrumpRx gives self-pay patients a coupon to show at a retail pharmacy, while NovoCare Pharmacy can ship to your door.
Ozempic, Novo's semaglutide for type 2 diabetes, has its own breakdown: Ozempic cost without insurance. If you're weighing a compounded version, see what compounded semaglutide cost and what changed after the shortage ended.
How we checked these prices
We read every price on the manufacturer's or the government's own page on 10/18/2026. Each table above lists those pages and the date. When a price changes, we date the correction on this page and add it to the price change log.
