The Zepbound price without insurance runs from $299 to $449 a month through LillyDirect, Lilly's own pharmacy service (checked 10/18/2026). That buys a 28-day supply of either the KwikPen or single-dose vials. The price rises with the strength.

The top tier comes with a condition. At 7.5 mg and above, the $449 price holds only if you refill within 45 days. Miss that window and the month costs $499 or $699.

One Zepbound form isn't sold this way at all. The single-dose pen is listed for insured buyers only.

How much does each Zepbound strength cost?

LillyDirect shows one self-pay price per strength. The KwikPen and the vials cost the same at every strength.

Each price covers 28 days. For the KwikPen, that's one multi-dose pen with four weekly doses. For vials, it's four single-dose vials.

Supplies cost extra. LillyDirect says the KwikPen needs a pen needle purchase, and vials need a needle and syringe purchase. It doesn't list what those cost, so we can't give you a number.

Lilly's page also notes that 2.5 mg is a starting dose and is not approved as a maintenance dose. That's Lilly's wording, not a buying tip. Which strength you use is a decision for you and your prescriber.

What happens if you refill Zepbound late?

You lose the $449 price on the four highest strengths. LillyDirect says those prices come through the Zepbound Journey Program, and regular prices apply if the prescription isn't refilled within 45 days.

At 10 mg and up, one late refill costs $250 more than an on-time one. The 2.5 mg and 5 mg prices carry no refill condition on the LillyDirect page.

The clock starts when your last order arrives. Lilly's KwikPen terms say the refill must be completed within 45 days of the delivery or received date of your previous fill. You get the $449 price automatically on your first purchase.

What did Zepbound vials cost before?

More. Lilly cut its vial prices on 12/01/2025, according to its news release:

$349 $299 2.5 mg, changed 12/01/2025 $499 $399 5 mg, changed 12/01/2025 $499 $449 7.5 to 15 mg, changed 12/01/2025

Today's KwikPen prices match those vial prices, strength for strength (checked 10/18/2026).

Is the KwikPen or the vial cheaper?

Neither. The price is identical at every strength. The differences are in how you buy and use each one:

  • The KwikPen is a multi-dose pen. LillyDirect sells it for self-pay with free delivery or pickup, and also says Select insurance accepted.
  • Vials are Exclusively available through LillyDirect and sold for self-pay only.
  • The KwikPen uses pen needles. Vials need a needle and syringe for each dose.

The Zepbound label (revised 08/2026) shows why the prices line up. A 2.5 mg KwikPen holds 10 mg of tirzepatide in 2.4 mL, split into four doses. Four 2.5 mg vials also add up to 10 mg.

There's one more gap, and it matters if you're on Medicare. The Bridge program covers the KwikPen but not vials. More on that below.

Can you buy the Zepbound single-dose pen without insurance?

Not from LillyDirect. Its single-dose pen listing shows an insurance price only: as little as $25 with Lilly's savings card.

That card needs a commercial plan that covers the pen. Lilly's terms cap the savings at $100 per 1-month fill and $1,300 a calendar year, and the card ends 12/31/2026. Medicare, Medicaid, TRICARE and other government plans are excluded.

Lilly itself says the single-dose pen could be the most cost‑effective option if your plan covers it and you qualify for the card. If your plan doesn't cover Zepbound, Lilly's card terms also offer the single-dose pen as low as $499 a month (checked 10/18/2026), but the KwikPen or vials are cheaper.

Can you get the cash price at another pharmacy?

Sometimes, with limits. Lilly offers a KwikPen self-pay card for use at other pharmacies, aimed at the same $299 to $449 prices. The card's savings are capped each month, though. The caps are $215 at 2.5 mg, $115 at 5 mg, $65 at 7.5 mg and $271 above that.

The cap decides what you pay. At 2.5 mg, $299 plus the $215 cap is $514. If a pharmacy charges more than $514, you pay the difference on top of $299. Through LillyDirect, Lilly says the savings apply automatically.

What is the Zepbound list price?

We couldn't confirm it from Lilly directly. Lilly's price disclosure site blocked our automated check on 10/18/2026.

The government's TrumpRx site shows a "before" price of $1,087 a month for Zepbound vials, the same at every strength (checked 10/18/2026). TrumpRx then sends vial orders to LillyDirect, so its price is LillyDirect's price.

The list price matters most if your plan bills you a share of it through coinsurance or a deductible. For a cash buyer on LillyDirect, it's mostly background.

Want to compare strengths by the milligram? Our tirzepatide cost per mg breakdown runs the numbers for every Zepbound and Mounjaro price. The diabetes version of the same drug has its own price check: Mounjaro cost without insurance.

Does a cash Zepbound purchase count toward your deductible?

No. Lilly's KwikPen self-pay terms say you agree not to apply what you pay toward your deductible or true out-of-pocket obligations. You also agree not to seek reimbursement from any insurance plan or healthcare reimbursement account.

LillyDirect labels each Zepbound form HSA/FSA eligible: Eligibility may vary by plan. Put those two together. The cleaner path is to pay with your HSA or FSA card at checkout, not to file a claim afterwards.

Does Medicare cover Zepbound?

One form, through a temporary program. The Medicare GLP-1 Bridge gives eligible Part D enrollees a $50 copay from 07/01/2026 to 12/31/2027. For commercial plans, see does insurance cover Zepbound.

CMS's fact sheet for prescribers lists the Zepbound KwikPen as covered when it's prescribed to reduce excess weight. It then adds: The single-dose Zepbound® pen and Zepbound® vials are NOT covered.

The Bridge has its own medical criteria, and your prescriber has to request it. People with type 2 diabetes, moderate-to-severe sleep apnea or fatty liver disease go through their Part D plan instead.

Before you pay

  1. Check whether your plan covers the Zepbound single-dose pen. If it does, the savings card may beat any cash price.
  2. If you're paying cash at 7.5 mg or higher, put your next order date in your calendar. A refill after 45 days costs $499 or $699 instead of $449.
  3. Budget for supplies. The KwikPen needs pen needles and vials need needles and syringes, and LillyDirect sells them separately.
  4. On Medicare Part D? Ask your prescriber whether the $50 Medicare GLP-1 Bridge applies. It covers the KwikPen only.
  5. Paying with HSA or FSA money? Use the card at checkout. Lilly's self-pay terms rule out claiming the cost back from a healthcare reimbursement account.
  6. Confirm the price on LillyDirect the day you order. Lilly says it can change or end these offers at any time.

How we checked these prices

We read every price on Lilly's or the government's own pages on 10/18/2026, and each table lists its sources with that date. Lilly's 12/01/2025 release is the one exception. Lilly's site blocked us, so we read it through a search-engine copy and note that in the source table. When a price changes, we date the correction here and add it to the price change log.
